(In some cases, your policy number may be the same as the clinic's.) Your policy number is a nine-digit number shown on all policy forms you receive from us. In most cases, the account number on your billing statement can be used interchangeably with your policy number.

Account numbers are located in the upper left and lower left portion of your billing statement or on the detail page of group billing statements.
