For the outpatient setting

Self-guided Risk Assessment Toolkit

The Self-guided Risk Assessment Toolkit is available for Members in the outpatient setting to independently assess areas of potential risk within their practice. The process is private and responses are confidential for Member use.

Self-guided Risk Assessment Toolkit: Three Simple Steps

Our Self-guided Risk Assessment Toolkit is intended to allow Members to privately assess their risks in the outpatient setting. The set of tools includes three steps: an anonymous online questionnaire, a template for medical record audits, and downloadable tips for best practices.

The anonymous nature of the Self-Guided Risk Assessment means that the risk consultants at Physicians Insurance will not have access to the results. If you would like support to address specific questions or risk exposures, please contact your risk consultant or email us for support. Our Risk Consultants are available to Members by phone Monday through Friday, 8:00 a.m. to 5:00 p.m. Pacific Time at (800) 962-1399 or via email at [email protected].

STEP 1: Complete the Confidential Online Survey

To begin your self-guided assessment, take the following confidential online survey and download the results for your use as an insightful benchmark. The survey covers topics such as consent, health information management, medication management, and patient safety.

STEP 2: Review and Implement Best Practices

Download the following PDF and save for future reference or internal distribution within your team. Our risk consultants have documented these risk management best practices for the outpatient setting based on the most commonly seen claims concerns and questions from Members.

STEP 3: Conduct an Outpatient Medical Record Audit

Use the following audit tool to guide you in performing initial and ongoing medical record audits at your organization.

This Excel file includes multiple worksheets that will assist you with documenting a thorough review. In addition to general topics, it includes audit templates for topics such as informed consent, the use of chaperones and interpreters, phone calls, closing the loop, as well as offering best practices and tips across key areas of risk that we commonly see.
